New York State Medicaid Waiver is a program intended to support Medicaid members who demonstrate health-related social needs in New York State.


Is Met Council a Service Provider?

Met Council is currently providing screening, navigation, and enhanced care management services in Brooklyn, Queens, Manhattan, Staten Island, and the Bronx. This includes assessment, referrals, and providing direct nutrition services, such as groceries and medically tailored meals.
