What is the 1115 Waiver?

New York State Medicaid Waiver is a program intended to support Medicaid members who demonstrate health-related social needs in New York State.

Met Council is currently providing screening, navigation, and enhanced care management services in Brooklyn, Queens, and Manhattan. This includes assessment, referrals, and providing direct nutrition services, such as groceries that meet the strictest kosher guidelines.
